2021 SPORTS PHYSICALS/IMMUNIZATION CLINIC
The Sanford Middle and High Schools are holding a sports physical and immunization clinic on Wednesday, July 28, 2021. Students entering 7th through 12th grade who need a sports physical, theirTDap and/or MCV4 (meningococcal) vaccines will be able to receive everything in one location!
Vaccines will be offered from 12PM to 4PM and sports physicals from 3PM to 5PM at Sanford High School in Room D105.

Enter SHS at the Community Retail Center entrance near the greenhouse. The cost for the Sports Physicals is $20. Make checks payable to York Hospital. The immunizations are free!. Students must be accompanied by their parent/guardian or have the Limited Sports Exam and/or Immunization Consent form signed by their parent/guardian.
All physicals are valid for two years from the date the physical was given.
